The Subcontractor will perform a visual Facilities Condition Assessment Survey (FCA) for the purpose of documenting the property and identifying the condition and material defects of the subject properties in accordance with ASTM E 2018-08, the Standard Guide for Property Condition Assessments: Baseline Property Condition Assessment Process, and additional assessments and in accordance with the Statement of Work (SOW) Exhibit I.
FRA will award a subcontract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to FRA in terms of price and other factors specified in this solicitation.
FRA may (a) reject any or all quotations, (b) accept other than the lowest quotation, and (c) waive informalities and minor irregularities in quotations received.
FRA may award a subcontract on the basis of initial offers received, without discussions. Therefore, each initial quotation should contain the offerors best terms from a cost or price perspective.
FRA requires all subcontractor employees and sub-tier subcontractors coming on to the Fermilab to comply with Real-ID Requirements. For more information, please go to: https://eshq.fnal.gov/realid/.
All employees working on- site must undergo FRA’s Contractor’s Safety Training and is required to be updated every two years. This training is offered Monday through Friday at 7:30 AM. The duration of the training is approximately 30 to 45 minutes.
Proposal Submission Format
The proposal shall contain the following documents/items:
1) Price Proposal Form
The proposed pricing shall be a firm fixed price for the work. This Price Proposal form is required to be completed and signed.
2) Subcontractor Annual Representations & Certifications (SARC), Form SARC
The successful Offeror’s submittal of this completed form will be incorporated in the resultant subcontract. If an offering entity is a joint venture, association, consortia, or partnership, each member entity must submit individual Representations and Certifications Forms.
3) Proposal Certifications, Form PUR-466
All appropriate sections are required to be completed and signed by a company official.
